Based on the care profile of Mr. James Warden. I think the most high risk five factors for his skin breakdown are the following with Explanation of risk and evidence-based intervention for prevention.
Immobility & Limited
Functional Status
Explanation of Risk: As we know that Mr James Warden is on wheel chair and dependent for his Activities of Daily Living. So somtime during the trasnfer process and long time on wheelchair and in the bed could results pressure injuries.
Evidence-Based Intervention: So to prevent those pressure injuries we can use proper transfer techniques, use cushions to minimize the shear. We can also schedule 2 hours in bed and 1 hour in wheel chair position change.
Medical Condition
(Diabetes)
Explanation of Risk: As mentioned in the care profile that Mr James have Diabetes which could effect of his blood circulation and causes neuropathy. It also effect on healing process and loss of sensation in his body. He can't shift his weight by him self which could make pressure injuries.
Evidence-Based Intervention: I will inspect the skin more fequently and check the sensations in all libs so that if any pressure injury he have he can inform a care taker verbaly.
Dependence on Assistance
for ADLs
Explanation of Risk: As Mr James have imputed leg he need help for taking bath and other activities so we know that access moisture in skin floding and peri area could damage the skin. Moisture makes skin more soft which increase the chances of friction injuries.
Evidence-Based Intervention:I will clean the all creases after each baath or more frequently to iradicate extra moisture due to sweat or bathing. Apply barrier cream to each folding or peri areas.
Aggression & Resistance
Explanation of Risk: Mr James daignose with bipolar disorder when he moves away while moving that could damage his skin by rubbing against bed sheet or wheel chair. Thta could make easy presure injury
Evidence-Based Intervention: I will try to calm himself before making any move and try to have enough staff to move him safly.
Nutritional Integration
Explanation of Risk: Mr James have health problem and he need more protien than noraml. If he didn't eat well then he his body will use fat and musels as it's fuel which could make major areas un protected and eaisy to expose presure enjuries.
Evidence-Based Intervention: To keep him safe I will take close eye on his diet which should have high in protien and encourage him for plenty of water which keep his skin hydrated and reduce the chances of pressure injuries.
