Explanation of Risk: As we know that Mr James Warden is on wheel chair and dependent for his Activities of Daily Living. So somtime during the trasnfer process and long time on wheelchair and in the bed could results pressure injuries.

Evidence-Based Intervention: So to prevent those pressure injuries we can use proper transfer techniques, use cushions to minimize the shear. We can also schedule 2 hours in bed and 1 hour in wheel chair position change.

Medical Condition
(Diabetes)

Explanation of Risk: As mentioned in the care profile that Mr James have Diabetes which could effect of his blood circulation and causes neuropathy. It also effect on healing process and loss of sensation in his body. He can't shift his weight by him self which could make pressure injuries.

Evidence-Based Intervention: I will inspect the skin more fequently and check the sensations in all libs so that if any pressure injury he have he can inform a care taker verbaly.

Aggression & Resistance

Explanation of Risk: Mr James daignose with bipolar disorder when he moves away while moving that could damage his skin by rubbing against bed sheet or wheel chair. Thta could make easy presure injury

Evidence-Based Intervention: I will try to calm himself before making any move and try to have enough staff to move him safly.
